New Jersey Goals of Care is proud offer a series of four webinars this fall that will cover decision making in geriatric medicine, an introduction to our Four-Step Model of care and use of the NJ POLST form. These webinars will be viewed during Internal Medicine resident noon conferences. These four noon conferences will be multidisciplinary, and we will offer CME and CE credits for physicians, nurses and social workers. To learn more please email us or speak to your facility’s Internal Medicine Residency Program Director. The webinars were made possible through a generous grant from The Horizon Foundation for New Jersey.
