There is a critical need to improve the quality of the health care provider-patient dialogue at the end of a person’s life. A grant from the Robert Woods Johnson Foundation was utilized to establish highly- targeted local initiatives to help educate health care providers and patients about the need for high quality communication about end-of-life care.
